OzChild's Support and Choice program enables adults with a disability to live independently.

Support and Choice facilitators assist people with a disability and their families/carers to create a plan that reflects the clients' dreams, desires and goals, and describes how to achieve them.

Support and Choice case managers assist people with a disability to achieve the goals of their Support and Choice plan.

A Support and Choice plan may include funding for case management, personal care, one to one support, accessing recreational community services and other desired dreams.
